A epidural either lumbar or caudal is safe. But preferable to avoid opioid including fentanyl as these children have reflux problem.
Clonidine can be added to LA drugs to prolong the analgesia.
A detailed case history about mile stones,severity of spasm etc if given, a case discussion is possible.
